Profhilo vs Profhilo Structura: What’s the Difference?

Profhilo and Profhilo Structura belong to the same injectable treatment family, but they are designed to address different aspects of facial ageing.

In simple terms, Profhilo primarily focuses on improving skin quality, hydration and laxity. Profhilo Structura works at a deeper level and is intended to support areas affected by changes within the superficial facial fat.

Neither treatment should be chosen simply because one is newer or sounds more advanced. The right option depends on what is causing the changes you can see, whether that is dehydration, thinning skin, reduced elasticity, changes in facial support or a combination of concerns.

My Expert View: The Difference Is About Tissue, Not Age

One of the most common misunderstandings I hear is that Profhilo is for younger patients and Profhilo Structura is for older patients.

I would not choose between them based on age alone.

Two people of the same age can have very different facial anatomy, skin quality and patterns of ageing. One may have dehydrated, crepey skin with relatively good facial support. The other may notice flattening through the cheeks, heaviness lower in the face or a change in facial shape caused by changes beneath the skin.

The most important question is not, “How old are you?”

It is, “Which layer of the face needs support?”

What Is Profhilo?

Profhilo is an injectable hyaluronic acid treatment indicated for tissue remodelling and improving skin laxity on the face and neck. It contains stabilised complexes of high- and low-molecular-weight hyaluronic acid produced using IBSA’s patented thermal technology.

It is often described as a skin-remodelling treatment because it is not injected to create a defined cheek, sharpen the jawline or replace volume in the same way as a traditional dermal filler.

In practice, I consider Profhilo when a patient’s main concerns include:

Profhilo spreads through the treated tissue rather than remaining as a deliberately shaped deposit. The aim is an overall improvement in skin quality rather than a visible change to facial structure.

Who May Benefit Most From Profhilo?

Profhilo may be appropriate for someone who feels their skin looks tired, dry or less resilient but does not necessarily need additional facial support.

It can be particularly appealing to patients who want subtle rejuvenation and do not want their features to look fuller or noticeably altered.

However, it has limitations. Profhilo is not designed to replace significant lost volume, reposition facial tissue or produce the same result as anti-wrinkle injections or dermal filler.

What Is Profhilo Structura?

Profhilo Structura is also made using high- and low-molecular-weight ultrapure hyaluronic acid, but it has a different formulation and clinical purpose.

Profhilo Structura is described as a facial treatment intended for restoring adipose tissue. This means it is designed to work with the superficial fat compartments that contribute to facial shape and support.

These fat compartments change as we age. They may become thinner, shift or provide less support to the overlying skin. This can contribute to flattening through the mid-face, changes in facial contour and increased heaviness further down the face.

Profhilo Structura is therefore more relevant when the concern is not just skin quality, but an alteration in the tissue beneath the skin.

Who May Benefit Most From Profhilo Structura?

I may consider Profhilo Structura when someone notices:

  • Flattening or reduced support through the cheek area
  • Early changes in facial shape
  • Skin that appears to be descending rather than simply becoming dry
  • Increased heaviness around the lower face
  • A tired or drawn appearance associated with changes in superficial fat
  • A preference for regenerative aesthetics rather than conventional volumising treatment

It should not automatically be described as a filler replacement. Although both products contain hyaluronic acid, the assessment, formulation, injection approach and intended tissue response are different.

Does Profhilo Structura Give More Lift Than Profhilo?

This is where careful wording matters.

Profhilo Structura may improve the appearance of support in an appropriately selected patient because it is designed to address a different facial layer. That does not mean it will provide a surgical lift or create a dramatic change.

When someone has significant skin laxity, substantial volume loss or marked tissue descent, neither Profhilo treatment should be presented as a complete solution.

Sometimes the safest and most honest advice is that a different treatment, a combination plan or no injectable treatment at all would be more appropriate.

My preference is always to explain what a treatment can realistically improve, as well as what it cannot.

Is Profhilo Structura Better For Mature Skin?

Profhilo Structura can be particularly relevant to mature skin, but “mature skin” is a broad description rather than a diagnosis.

A patient may have excellent facial support but noticeable dehydration and thinning skin. In that situation, original Profhilo could be the more logical choice.

Another patient may have relatively good skin hydration but clear changes in the superficial fat compartments. Profhilo Structura may then be more relevant.

A third patient may have both concerns and could potentially benefit from a staged or combined approach, provided this is clinically appropriate.

The product should follow the assessment. The assessment should never be made to fit the product.

Can Profhilo And Profhilo Structura Be Used Together?

They may form part of the same longer-term rejuvenation plan because they address different layers and concerns.

That does not necessarily mean they should be administered at the same appointment or that every patient needs both.

I generally favour a staged approach to regenerative aesthetics. This allows time to assess the response to each treatment, avoid unnecessary intervention and make decisions based on how the tissue changes rather than following a predetermined package.

How Do They Compare With Dermal Filler?

Traditional dermal fillers are normally selected when the objective is to replace volume, define a feature, improve proportion or provide targeted structural support.

Profhilo is not designed to create that type of shape or projection.

Profhilo Structura works more structurally than original Profhilo, but it should not be assumed to behave like a conventional filler. Its purpose is linked to the quality and condition of adipose tissue rather than simply filling a hollow or building a contour.

For patients who are worried about looking overfilled, this distinction can be reassuring. However, avoiding filler at all costs is not always the correct clinical decision. A small amount of precisely placed filler may sometimes address a concern more effectively than repeated treatments that were not designed for it.

The best treatment is the one suited to your anatomy, not the one with the most appealing label.

What I Look For During A Consultation

Before recommending either treatment, I would assess more than the area a patient points to in the mirror.

For many people, preserving their natural expressions and avoiding a fuller appearance is just as important as improving the concern that brought them to the clinic.

That conversation often determines the treatment plan more effectively than focusing on an individual product.

My Advice If You Are Unsure Which Treatment You Need

Avoid deciding from photographs, social media videos or another person’s result.

Before-and-after images can be helpful, but they do not show the full assessment, the patient’s anatomy, lighting differences, previous treatments or whether several procedures were used.

Instead, begin by describing the change you have noticed.

For example:

“My skin feels thinner and looks crepey.”

“My cheeks look flatter than they used to.”

“My lower face feels heavier.”

“I look tired even when I am well rested.”

“I want healthier-looking skin without changing my face.”

Those descriptions provide more useful clinical information than arriving convinced that you need a particular product.

Is Profhilo Structura A Dermal Filler?

It contains hyaluronic acid, but it is not designed to create defined projection or sculpt facial features in the way traditional dermal fillers do. Its intended focus is the restoration of facial adipose tissue.

Will Either Treatment Change The Shape Of My Face?

Original Profhilo is intended to improve skin laxity and tissue quality rather than reshape the face. Profhilo Structura may subtly improve support where superficial fat has changed, but the aim should remain natural and proportionate.

Which Treatment Is Better For Crepey Skin?

When crepey texture, dehydration and reduced elasticity are the main concerns, original Profhilo may be the more relevant option. A consultation is still necessary because similar-looking concerns can have different underlying causes.

Which Treatment Is Better For A Flat Or Tired-Looking Mid-Face?

Profhilo Structura may be considered when changes in the superficial facial fat are contributing to reduced support or flattening. It will not be suitable for every type or degree of volume loss.

Can Profhilo Structura Replace A Facelift?

No. Injectable treatments cannot reproduce the repositioning achieved through surgery. Any suggestion that Profhilo Structura provides a surgical-equivalent lift should be treated cautiously.

Are Profhilo Treatments Suitable For Everyone?

No injectable treatment is appropriate for everyone. Suitability depends on medical history, pregnancy or breastfeeding status, active infection, allergies, previous treatments, anatomy and expectations. A qualified practitioner should assess these factors before treatment.

So, Which One Would I Choose?

If the main problem is skin quality, dehydration or mild laxity, I would be more likely to discuss Profhilo.

If the concern appears to come from changes in superficial facial fat and reduced support, I may discuss Profhilo Structura.

When both layers are involved, a carefully staged combination may be appropriate.

And when neither treatment is likely to provide the result someone wants, I would explain that honestly.
